Your Members do not experience health issues, acute or chronic, in isolation. Non-medical issues (often called “social determinants of health”) can greatly affect the Member’s ability to comply with a plan of care and impact quality and cost outcomes. Research shows that social determinants like food insecurity, housing insecurity, transportation limitations and substance misuse make the difference between an individual who can manage a chronic condition or become a high utilizer.

This presentation will briefly address social determinants and their relevance in today’s risk environment and then delve into issues arising from arrangements targeted to social determinants interventions.  You will learn: 

  • The various systems being used to address social determinants;
  • Important factors in relationships between payers and social service agencies, privacy issues, contracting issues, closed referral loops and performance standards
  • Ways in which social services agencies can “braid” income streams by leveraging other payment sources.
  • Does it work? Determining Return on Investment for interventions
